The Swim Alberta Sport Accident Insurance Program is designed to provide benefits to Swim Alberta members, should a member be injured as the result of an accident, during a sanctioned and properly supervised Swim Alberta event.

The insurance program does not require negligence to be proven before benefits are paid.  As long as the accident occurs within the provisions of the program, benefits will be paid.

Who is Covered Under the Plan?
Swimmers including associate and rookies (while “trying-out” - 2 week maximum at the beginning of the season), who are registered with Swim Alberta. Coaches, trainers, officials, chaperons, parent volunteers, general members, and auxiliary workers "acting in an official capacity", on behalf of a member club of Swim Alberta. 

When is Coverage in Effect?
Participants are covered during Provincial training camps, sanctioned swim meets, practices and training sessions, under the supervision of proper authority of the member club.  This includes travel directly to and from meets and practices, provided there are more than two familites in the vehicle.

Coverage is in effect as long as your swim club remains in good standing with Swim Alberta.

Tuition up to $5,000
Special Treatment Travel up to $2,500 
Out of Province up to $10,000
Eyeglass, contact lens up to $200(resulting from injury)
Emergency transportation up to $100
Blanket Dental up to $5,000(including 75% future)
Dentures/Bridgework up to $2,500
Fractures up to $500
Babysitting up to $500
Youth Wage Loss up to $1,000

1. Loss of Life, Loss of Sight, Dismemberment
-  Loss of sight, speech or hearing or loss of limb/s only one per accident, which ever greatest
-  Loss of life must occur within 90 day of accident
-  Prosthetic Devices

2. Blanket Accident Expenses
-  Insurer will pay for medical expenses, not covered under the Government plan within 52 weeks of the accident and 30 days for services or supplies while under care of a physician.

3. Rehabilitation Benefit
- Coverage for special training is available in order to retrain an individual to allow them to engage in a special occupation as a result of the injury.

4. Special Treatment Travel Expenses- If treatment is required and can’t be obtained within the municipality, $150 per injured person per claim for travel expenses is available. 

5. Out of Province Surgical/Medical Accident
-  Expenses eg. Surgical operation & hospital in excess of your provincial coverage and/or medical plan, is available for an injury sustained outside of province but within Canada 

6.   Eyeglass and Contact Lens (resulting from injury)
-  Breakage of eyeglasses or loss/breakage of contact lens, the actual cost of repair/replacement is covered 

7.   Emergency Transportation
-  Offset cost for transportation (by licensed taxi only) to nearest hospital, doctor's office and return home. 

8.  Blanket Dental Reimbursement
- Injury to whole and sound teeth due to an external blow to the mouth, which requires x-rays, treatment, and replacement
- Expenses not to exceed benefits schedule 

9.  Dentures/bridgework
- Treatment by a physician and/or dentist within 30 days of the accident is covered.
- Repair or replacement not to exceed stated Schedule of Benefits

10.   Babysitting
- If a youth member is confined to their home under a physicians care, following the accident, during normal school hours and the parent is unable to provide care.

11. Youth Wage Loss
- 75% of the hourly wage is covered during disability if under a physicians care 

NOTE: Benefits under the Accident Medical and Dental Expense Reimbursement sections of this policy will be reduced by any amount paid or payable under any other policy providing similar reimbursement expenses held by members. 

Additional Benefits 

1.Chaperons and Voluntary Team Officials
Coverage is provided to chaperons and team officials volunteering their time and expertise at organised functions.

2.Out of Country Hospital and Medical
The Out of Country accident coverage provides reimbursement for medical and dental expenses incurred as the result of an injury which occurs while participating in a Swim Alberta sanctioned event out of country. This plan requires payment for expenses, up front with reimbursement upon return. Maximum reimbursement varies depending on treatment required. There is additional expense for this coverage. 

Process:
A club must contact Swim Alberta for an insurance quote. Premium will depend on the number of participants and days out of country. Forms are available through the Swim Alberta office. Request must be presented 2 weeks prior to travelling.

Limitations & Exclusions

No benefits are payable for loss due to:
- Sickness or disease either as a cause or effect
- Benefits covered under Alberta Health Care
- An act of war
- An intentional self inflicted injury while sane or insane
- Use of experimental drugs not approved by Health Protection Branch
- Any medial services eg. Athletic sports therapy, chiropractic, and nursing, physiotherapy, when employed or engaged by the policyholder.
- Riding as a passenger in any air vehicle other than provided under Aircraft Coverage section.
- Dental services other than provided under Dental Expenses
- No benefits are payable if the member is a professional athletes

Claim Procedures
- Written notice of injury must be submitted within 30 days of the date of accident causing such injury.
- Claim forms are available from Swim Alberta
Forms should be completed in full and returned to Swim Alberta.

Liability Insurance

General:
Third Party coverage is available for Swim Alberta registered clubs up to $5,000,000 per occurrence arising from negligence. Protection is provided for directors, employees, members, volunteers, coaches, managers, and chaperones. The policy provides legal defence funds, when legal action arises from events covered by the policy. 

Personal Injury:
Coverage is available for actions by a third party, against the association/club for personal injury including false arrest, malicious prosecution, wrongful detention, invasion of privacy, wrongful eviction or entry, libel, slander, humiliation and defamation of character. 

Non-owned Auto:
Covers the association/club, if it is held legally liable for damages arising out of the use of non-owned vehicles on association business. Third party protection is provided to the association/ club for employees or volunteers using their own vehicles on association/club business. Legal fees included. 

Medical:
Covers third party medical payments up to $1,000.00 per person to a total amount of $10,000.00

Certificate of Insurance
A Certificate of Insurance may be required for clubs holding fundraising activities, training camps, etc., outside Alberta swimming pools. This will be dependant on the venue rules. Depending on the activity there could be an additional premium required.

FOR BOTH Medical and Liability coverage, a Notice of Event form must be supplied to the Swim Alberta three (3) weeks prior to the activity, to ensure coverage. Forms can be obtained through the Swim Alberta office, off the Swim Alberta web site, or in the Swim Guide.
