Description of forms (Not all forms will be required by all clubs)

Club Affiliation Form: Must be submitted to Swim Alberta no later than 30 September 2008 and be accompanied by the affiliation fee. This provides all pertinent contact information required by our office. Associated fee = $300.00.

On Line Waiver Form: Must be submitted prior to acquiring access to the Swim Direct registration on line system. Please print legibly.

Freedom of Information & Protection of Privacy Act/Personal Informaton Protection Act: All parents/guardians/swimmers (if over the age of majority) must complete this form. All forms must be kept by the club’s registrar UNLESS consent is not given for full use of information. If consent is not given then the original form must be sent to Swim Alberta and a copy kept with the club.

Alberta Coaches Council: Registration of coaches must be completed through the Canadian Swim Coaches & Teachers Association web site, http://csca.org. Swimming Canada coach membership is completed using on line registration system.  Contact your club registrar to begin the process. 

All person acting in the capacity of a swim coach in the province of Alberta (paid or volunteer), must be members of the Alberta Coaches Council. Fees are: $30.00 per coach. 

These persons must also have a completed police background check through http://www.mybackcheck.com/swimalberta prior to 1 January. This is an annual requirement.

They must also have successfully completed either the NCCP Theory and Technical Level 1 courses or the Skills Coach course prior to 1 January.

Sanction Application: All competitions must be sanctioned by Swim Alberta. Sanctions must be completed using the Swimming Canada meet sanctioning/results web site, http://www.swimming.ca/meetlist.aspx.

Time lines and requirements are available in the officials section of the Swim Alberta web site, titled Competitions Guidelines. Associated fee = $100.00 per sanction (only if an application is late).

IPC Sanction Application:  All competitions allowing swimmers with a disability to compete must download and submit the 'Application for Competition Approval' form to the International Paralympic Committee

Unattached Masters Registration Form:  Unattached masters will be registered through the Swimming Canada on line registration site.  Please complete the forms in full and return to our office accompanied by the $37.00 registration fee.

Insurance Forms:

Notification of Claim - Athletics Group Department: This form must be completed for any accident. Written notice of injury must be submitted within 30 days of the date of the accident. Claim form must be completed in full and returned to Swim Alberta. If any bills are to be claimed, original copies of these bills must be attached.

SportsCan Insurance Consultants Other Insurance Declaration Form: This form must accompany the Notification of Claims form.

Notice of Event - Insurance Request: This form must be submitted for any event or activity which registered swimmers and coaches participate in other than Swim Alberta sanctioned events/ competitions. A Certificate of Insurance might be requested by facilities being rented for fundraising activities (hot dog sales, floats, year end parties, etc.). A period of 3 weeks must be allowed for processing of request by the insurance company. Additional medical insurance can be purchased through Swim Alberta for out of country trips at the rate of $1.50 per participant x the number of days away or for extra activities, which quotes would need to be obtained. Out of Province Travel Insurance Request Form: This form must accompany the Notice of Event form with the names of the travelling participants (swimmers, coaches, chaperones) and their dates of birth.

Swim-A-Thon Forms:

Swim-A-Thon Request Form:  Must be submitted to Swim Alberta prior to Swim-A-Thon supplies being sent to the club.  

Participant Certificate:  Given to all participants who are involved in the clubs Swim-A-Thon.

Pledge Sheet:  Participants use this form to keep track of persons who sponsor them for the Swim-A-Thon.
